Orofacial myofunctional speech-language-hearing teletherapy program focusing on chewing and swallowing in temporomandibular disorders

Programa de terapia miofuncional orofacial com enfoque na mastigação e deglutição, por meio da telefonoaudiologia, na disfunção temporomandibular

Abstract

Purpose: to propose a myofunctional speech-language-hearing teletherapy program focusing on chewing and swallowing for individuals with temporomandibular disorder.

Methods: the first stage included a scoping review, which found strategies that served as a basis for creating the program. In the second stage, a speech-language-hearing pathologist specializing in oral motor therapy applied the initial version of the therapeutic protocol to 29 patients diagnosed with temporomandibular muscle dysfunction, seeking to verify the instrument’s applicability. In the third stage, three speech-language-hearing pathologists specializing in oral motor therapy analyzed the protocol and suggested changes.

Results: the version created from the scoping review was applied to 29 participants, making no adjustments in this stage. All format modifications suggested by the specialists were accepted, whereas content changes were accepted when at least two participating professionals agreed, who suggested minor changes. The final protocol version had 14 sessions - the first and last for assessments and the rest for therapy.

Conclusion: the program was created, adjusted, and presented, aiming to assist and guide professionals in orofacial myofunctional speech-language-hearing teletherapy with this population. Further research is greatly important to validate this instrument.

Resumo

Objetivo: apresentar a proposta de um programa de terapia miofuncional com enfoque na mastigação e deglutição por meio da telefonoaudiologia para indivíduos com disfunção temporomandibular.

Métodos: na primeira etapa, uma revisão de escopo foi realizada e as estratégias encontradas serviram como base para a criação do programa. Na segunda etapa, a versão inicial do protocolo terapêutico foi aplicada por uma fonoaudióloga, especialista em Motricidade Orofacial, em 29 pacientes diagnosticados com disfunção temporomandibular muscular, buscando verificar a aplicabilidade do instrumento. Na terceira etapa, o protocolo foi analisado por três fonoaudiólogas especialistas em Motricidade Orofacial, as quais sugeriram alterações.

Resultados: a versão criada a partir da revisão de escopo foi aplicada em 29 participantes e, durante essa etapa, não foram realizados ajustes. Todas as modificações de formato sugeridas pelas especialistas foram acatadas. As alterações de conteúdo foram aceitas quando havia concordância entre, pelo menos, dois profissionais participantes. Mínimas alterações foram sugeridas. A versão final do protocolo incluiu 14 sessões de atendimento, sendo a primeira e a última de avaliações e as demais de terapia.

Conclusão: o programa foi criado, ajustado e apresentado com o objetivo de auxiliar e nortear os profissionais na terapia miofuncional orofacial por meio da telefonoaudiologia junto a essa população. É de suma importância que, em uma pesquisa futura, este instrumento seja validado.
