Unilateral hearing loss: benefit, satisfaction, and use of hearing aids

Perda auditiva unilateral: benefício, satisfação e uso de prótese auditiva

Abstract

Purpose: to investigate speech recognition in silence and noise of subjects with unilateral hearing loss (UHL), with and without hearing aids, and to analyze the benefit, self-perception of functional performance, satisfaction and use of hearing aids in this population.

Methods: an anamnesis was applied, the Sentence Recognition Thresholds in Silence and Noise (SRTS and SRTN) and the Percentage Indexes of Sentence Recognition in Silence and Noise (PISR and PISR) were obtained in subjects with UHL, without and with hearing aids. The Speech, Spatial and Qualities of Hearing Scale (SSQ) questionnaire and the Satisfaction With Amplification in Daily Life (SADL) were also applied to verify hearing aid use time.

Results: the SRTS, when speech occurred on the side of the worse ear, was higher than what is observed in subjects with normal hearing. The signal/noise (S/N) ratios were higher than expected when compared to normal hearing subjects, both when speech and noise were presented frontally and when speech was presented frontally and noise was presented on the side of the better ear. Hearing aid use provided benefits in all evaluated positions. The subjects reported few limitations in communication, despite their main complaint being related to the difficulty in recognizing speech in different communicative situations. The subjects were satisfied with the sound amplification provided by the hearing aid. Most of the subjects did not effectively use the hearing aid.

Conclusion: the use of hearing aids has provided benefit in speech recognition in silence and in noise. Even though the subjects presented benefit and satisfaction with sound amplification, most did not make effective use of the hearing aid. Discontinuation of use can be justified by the difficulty of perceiving the limitation of participation, as well as the benefit of the hearing aid in everyday life, in addition to concern about the costs of batteries, technical problems and aesthetic aspects.

Resumo

Objetivo: investigar o reconhecimento de fala no silêncio e no ruído de sujeitos com perda auditiva unilateral (PAUn), sem e com prótese auditiva, e analisar o benefício, autopercepção do desempenho funcional, satisfação e o uso de prótese auditiva nesta população.

Métodos: foi aplicada uma anamnese, obtidos os Limiares de Reconhecimento de Sentenças no Silêncio e no Ruído (LRSS e LRSR) e Índices Percentuais de Reconhecimento de Sentenças no Silêncio e no Ruído (IPRSS e IPRSR) em sujeitos com PAUn, sem e com prótese auditiva. Foi aplicado o questionário Speech, Spatial and Qualities of Hearing Scale (SSQ) e o Satisfaction With Amplification in Daily Life (SADL), além da verificação do tempo de uso da prótese auditiva.

Resultados: os LRSS, quando a fala incidiu do lado da pior orelha, foram mais elevados do que o observado em sujeitos com audição normal. Foram necessárias relações sinal/ruído (S/R) mais elevadas em relação ao esperado em sujeitos normo-ouvintes, tanto quando a fala e o ruído foram apresentados frontalmente, como quando a fala foi apresentada frontalmente e o ruído do lado da melhor orelha. O uso da prótese auditiva proporcionou benefícios em todas as posições avaliadas. Os sujeitos referiram poucas limitações na comunicação, apesar da principal queixa estar relacionada à dificuldade de reconhecimento de fala em diferentes situações comunicativas. Os sujeitos demonstraram-se satisfeitos com a amplificação sonora proporcionada pela prótese auditiva. A maioria dos sujeitos não fazia uso efetivo da prótese auditiva.

Conclusão: o uso da prótese auditiva proporcionou benefício no reconhecimento de fala no silêncio e no ruído. Mesmo que os sujeitos apresentassem benefício e satisfação com a amplificação sonora, a maioria não fazia uso efetivo da prótese auditiva. A descontinuidade do uso pode ser justificada pela dificuldade de perceber a limitação de participação, assim como o benefício da prótese no dia a dia, além da preocupação com os custos com as pilhas, problemas técnicos e aspectos estéticos.
